ABSTRACT

Objective:To compare the clinical outcomes between replacement with a composite press-fit radial head prosthesis versus open reduction and plate-screw internal fixation in the treatment of adult Mason Ⅲ radial head fractures. Methods:The clinical data of 64 adult patients with Mason Ⅲ radial head fracture were retrospectively analyzed who had been admitted to Department of Orthopedics, Beijing Friendship Hospital from January 2012 to December 2019. They were 18 males and 46 females, aged from 32 to 58 years (average, 45.7 years). They were divided into 2 groups: 32 cases received mini-plate-screw internal fixation (internal fixation group) and 32 cases replacement with a composite press-fit radial head prosthesis (replacement group). At the last follow-up, elbow valgus angle, range of elbow motion, Mayo score of elbow function and visual analogue scale (VAS) pain score were recorded and compared to evaluate the postoperative clinical outcomes. Results:There was no statistically significant difference in the preoperative general data between the 2 groups, showing they were comparable ( P>0.05). All patients were followed up for 8 to 48 months (average, 18.7 months). The operation time was, respectively, (81.4±8.2) min and (68.9±7.3) min for the internal fixation group and the replacement group, showing a statistically significant difference ( P< 0.05). For the internal fixation group and the replacement group at the last follow-up, the flexion and extension angles were 95° to 125° and 100° to 140°, the rotation angles 135.3°±11.2° and 143.5°±12.8°, and the Mayo scores 79.2±3.8 and 83.4±3.9, all significantly favoring the replacement group ( P<0.05). The VAS pain scores before operation and at the last follow-up were 7.6±0.7 and 0.9±0.7 for the internal fixation group, and 7.9±0.8 and 0.7±0.6 for the replacement group, showing significant differences between preoperation and the last follow-up in both groups ( P<0.05). All the incisions healed by the first intention, with no postoperative infection. Internal fixation loosening with ulnar neuritis was reported in one case in the internal fixation group; peri-prosthesis absorption with no prosthesis loosening was observed in one case in the replacements group. Conclusion:In the treatment of adult Mason Ⅲ radial head fractures, although both replacements with a composite press-fit radial head prosthesis and open reduction and plate-screw internal fixation can lead to satisfactory results, the former may be more effective.

ABSTRACT

Objective:To investigate the effectiveness of balloon-assisted technique for the treatment of intraprocedural aneurysmal rupture (IAR) during intracranial aneurysm coil embolization and its impact on the clinical outcomes of patients. Methods:Patients with intracranial aneurysm received coil embolization and complicated with IAR in Xijing Hospital of Air Force Medical University from January 2013 to January 2019 were enrolled retrospectively. They were divided into balloon-assisted hemostasis group and rapid packing hemostasis group according to the methods of intraoperative hemostasis. The modified Rankin Scale was used to evaluate the clinical outcomes at 3-month postoperative follow-up. A score of 0-2 was defined as a good outcome. Multivariate logistic regression analysis was used to identify the independent influencing factors of clinical outcome. Results:A total of 77 patients with IAR were enrolled, of which 46 (59.74%) used balloon-assisted hemostasis, and 31 (40.26%) used rapid packing hemostasis. In 51 patients (66.23%) with 3-month follow-up data, 32 (62.75%) had good outcomes, and 19 (37.25%) had poor outcomes. Univariate analysis showed that there were significant differences in time from IAR to treatment, time from IAR to confirmed hemostasis, postoperative Fisher grade changes, and good outcomes between the balloon-assisted hemostasis group and the rapid packing hemostasis group (all P<0.05). There were significant differences in IAR treatment methods, time from IAR to treatment, time from IAR to confirmed hemostasis, and postoperative Fisher grade changes between the good outcome group and the poor outcome group (all P<0.05). Multivariate logistic regression analysis showed that balloon-assisted hemostasis (odds ratio 0.234, 95% confidence interval 0.056-0.990; P=0.048) and time from IAR to confirmed hemostasis ≤10 min (odds ratio 0.097, 95% confidence interval 0.024-0.397; P=0.001) were the independent protective factors of the good outcomes in patients with IAR. Conclusion:Using balloon-assisted technique to treat IAR during intracranial aneurysm coil embolization can achieve satisfactory hemostatic effect and improve the clinical outcomes of patients.

ABSTRACT

Objective To quantitatively analyze the operation risks for femoral intertrochanteric frac-tures under the guidance of Acute Physiology and Chronic Health EvaluationⅡ( APACHEⅡ) . Methods A retrospective analysis was performed among the 226 patients with femoral intertrochanteric fracture who had re-ceived surgery between January 2013 and January 2016. They were 59 men and 167 women, aged from 69 to 106 years ( average, 75. 4 ± 6. 1 years ) . Their average APACHEⅡscore was 18. 3 ± 6. 3 ( from 5 to 34 ) . They were divided into 3 groups according to their APACHEⅡscores: 127 cases in the low risk group (≤ 15 ) , 68 cases in the medium risk group ( from 16 to 24 ) and 31 cases in the high risk group ( ≥25 ) . The mortality was compared between the 3 groups. The deaths and survivals were compared in each group in terms of age, oper-ation time, intraoperative blood loss, postoperative blood transfusion and combined internal diseases. Results Thirteen patients died during hospitalization, giving a mortality of 5. 7% ( 13/226 ) . The causes for death were heart attack in 6 cases, respiratory failure in 4, toxic shock in 2 and renal failure in one. The mortality in the high risk group ( 22. 6%, 7/31 ) was significantly higher than in the low risk group ( 1. 6%, 2/125 ) and in the medium risk group ( 5. 9%, 4/68 ) ( P <0. 05 ) . In the low risk group, the intraoperative blood loss was sta-tistically different between deaths and survivals ( P <0. 05 ); in the medium risk group, the intraoperative blood loss and postoperative blood transfusion volume were statistically different between deaths and survivals ( P <0. 05 ); in the high risk group, the age and intraoperative blood loss were statistically different between deaths and survivals ( P <0. 05 ) . Conclusions APACHEⅡcan be used to quantitatively evaluate the patients with femoral intertrochanteric fracture who usually suffer from intraoperative complications and concomitant in-ternal diseases. The most significant risk factor may be intraoperative blood loss.

ABSTRACT

Objective To study the effects of semaphorin 5A (SEMA5A) gene silencing by lentivirus?mediated short hairpin RNA(shRNA)on biological activity of malignant melanoma cell line A375. Methods Two pairs of interference sequences for SEMA5A gene(shRNA1 and A375?shRNA2)and a pair of control interference sequences were designed to build lentiviral vectors, which were then transfected into HEK293T cells to gain lentivirus. A375 cells were divided into three groups:experimental group(A375?shRNA1 and A375?shRNA2 cells)transfected with the lentivirus containing shRNA1 or shRNA2, negative control group (A375?con cells) transfected with that containing the control shRNA, and blank control group(A375 cells)receiving no transfection. The A375 cells with stable knockdown of SEMA5A gene expression were screened by puromycin. Subsequently, reverse transcription?PCR and Western?blot analysis were performed to detect mRNA and protein expressions of Semaphorin 5A in these cells, and methyl thiazolyl tetrazolium(MTT)assay was applied to evaluate the growth of cells. The scratch assay and invasion assay were conducted to estimate migration and invasion ability of cells. Results The lentivirus containing the SEMA5A?targeting shRNAs or control shRNA was successfully transfected into A375 cells, and stably transfected cells were gained after puromycin selection. The expressions of semaphorin 5A mRNA and protein in the A375?shRNA2 cells were significantly reduced compared with those in the A375?con and A375 cells(all P 0.05). The scratch assay showed that there was no obvious cell migration into the scratch in the experiment group, whereas the scratch was almost covered by cells in the negative control group and blank control group. The invasion assay showed that the number of A375?shRNA2 cells passing through the Transwell chamber was significantly smaller than that of A375 and A375?con cells(both P 0.05). Conclusion The silencing of SEMA5A gene by lentivirus?mediated shRNA could effectively down?regulate the expression of semaphorin 5A, and inhibit the growth, invasion and migration of A375 cells.

ABSTRACT

Objective To analyze the causes of complications and death of the femoral intertrochanteric fracture during perioperative period, and to assess the orthopedics surgery risk of aged. Methods 499 of 547 cases were retrospectively re?viewed (132 male, 367 female), who underwent operation from May 2007 to Jun 2014 in our hospital. Their age varied from 59 to 103 years old, with an average of 78.3 ± 7.3 years old. According to Evans classification, there were 84 cases of type I, 67 type II, 187 type III, and 161 type IV. The causes of death were analyzed and discussed. The factors including health assess?ment before operation, age, operating time and blood loss were studied statistically to find out their correlation with mortality of operation. Results There were altogether 354 complications, including pulmonary infection, urinary system infection, DVT, and cardiac events. There were 30 cases of death and the mortality was 6.0%. The major causes of death included infectious shock, pulmonary infection, cardiac events, kidney failure and hemorrhagic shock caused by hepatic rupture. Statistical results from the score of APACHEⅡshowed the factor which definitely influenced the mortality was the amount of blood loss during op?eration.APACHEⅡscoring area under the ROC curve was 0.938, and the best cutoff value was 17.5 (about an index of 0.767), with the sensitivity of 86.7%and specificity of 90%. The area of risk of death under the ROC curve by APACHE Ⅱscore was 0.817, the best cutoff value of 16.765%(about an index of 0.483), and the sensitivity and specificity of 93.3%and 55.0%. Conclu?sion There were a lot of complications and accompanied diseases when femoral intertrochanteric fractures happened in aged pa?tients. The main cause of death was septic shock. There were relationships between hemoglobin and the operating time and the de?gree of trauma. What’s more, it was confirmed that the APACHEⅡscoring system was very useful to value the condition of the patients with femoral intertrochanteric fractures, which can be recommended.

ABSTRACT

Postmenopausal osteoporosis is one of the commonest systemic bone metabolism diseases among menopausal women, mainly caused by lowering internal estrogen. Although Hormone Replacement Therapy (HRT) is an effective method in clinical practice for years, it shows side-effect in increasing gynecological carcinoma. It has already been proved by clinical tests that multiple traditional Chinese medicine formulas and their monomer ingredients and phytoestrogen-like active constituents contained in traditional Chinese medicines are effective on treating osteoporosis with relatively less side-effects comparing with HRT. They show protective and therapeutic effects by acting on estrogen receptors of targeted tissues and targeted cells and then affecting expressions of bone metabolism-related regulatory proteins and factors in downstream signal conduct paths. Recent studies on estrogen related receptor (ERR) provide new possibilities and pathways for mechanism of traditional Chinese medicine and their active constituents in osteoporosis.

ABSTRACT

The bioactive principles contained in blueberries (Vaccinium) are various kind of anthocyanins (anthocyanidins, or phenolic aglycone, conjugated with sugar), chlorogenic acid, flavonids, alpha-linolenic acid, pterostilbene, resveratrol, and vitamins. After oral administration, anthocyanins can pass through blood-brain barrier and thus appear in various organs and brain. Improve visual function by increasing rhodopsin regeneration and ocular health is the earliest reported bioactivities of anthocyanin. Recent studies demonstrated the benefit of blueberries to prevent the age-related chronic diseases such as cancer, diabeties, hyperlipidemia, hypertension, neurodegeneration, obesity, and osteoporosis through its apoptosis, antioxidant, antiinflammation, and antiangiogenesis effects. Blueberries can eradicate microorganisms for the prevention of symptomatic urinary tract infections in women. Thus, blueberries are recognized as one of the most nutritious foods and cultivated worldwide. However, how to prolong the shelving time of fresh fruit, well utilize the leaf and stem to isolate the bioactive chemicals, improve quality consistency of juicy and dry products, all should be further concerned.

ABSTRACT

<p><b>OBJECTIVE</b>To study the phytoestrogenic effects and their possible mechanisms of Jiaoai tang and Shenqi Jiaoai tang through the tests in mice and ER (+) MCF7 cells.</p><p><b>METHOD</b>Sixty kunming mice weighing 9-12 g were randomly divided into 6 groups: solvent control group (administrated equal dose of diswater), diethylstilbestrol control group (administrated diethylstilbestrol at a dose of 0.35 mg x kg(-1) x d(-1) and 4 Chinese medicine treated groups (administrated low or high doses of Jiaoai tang and Shenqi Jiaoai tang at 2.5 g x kg(-1) x d(-1) or 5.0 g x kg(-1) x d(-1) respectively). After administration for 4 days, the mice were sacrificed; uterus was removed and weighed, uterus rate was calculated. The blood serum was also separated. The proliferation rate of MCF7 cells influenced by Jiaoai tang and Shenqi Jiaoai tang was determined by MIT assay. PS2, ERalpha and ERbeta mRNA expression was quantified by Real-time PCR assay. Estrogen receptor antagonist ICI182, 780 was employed as a tool.</p><p><b>RESULT</b>Administration of Jiaoai tang and Shenqi Jiaoai tang at high dose significantly increased uterus rate in mice (P < 0. 05). The pharmacological serum from two high-dosage groups of Chinese herbal medicine decoction significantly enhanced proliferation rate of MCF7 cells (P < 0.05 or 0.01), while their effects were blocked by ICI182, 780 (P < 0.05 or 0.01). The pharmacological serum could cause elevation of pS2 level (P < 0.01) which would be obviously inhibited by ICI182, 780 (P < 0.01). ERalpha and ERbeta mRNA levels were also elevated significantly (P < 0.05 and P < 0.01 respectively).</p><p><b>CONCLUSION</b>Jiaoai tang and Shenqi Jiaoai tang have phytoestrogenic effects, which were attained via ER pathway. They can also increase the mRNA levels of estrogen receptor subtypes, especially ERbeta.</p>

ABSTRACT

Objective To study the clinical features and treatment of gingiva plasma cell granulo-ma.Method The clinical information of 9 cases with gingival plasma cell gmnuloma Was analyzed retre-spectively.Results The disease most frequently attacked the people between age 20-40 years(77.8%) and the main pathogenic position were at the gingival of bicuspid and molar teeth.Clinical maJlif.estation were gingival swelling and bleeding, dental loosening and dropping.Three patients were given focal resection. Subgingival curettage and Chinese traditional medicine. After loosening teeth found and peridental absortion shown in X-rays,6 patients were operated to extract loosening teeth and reseet their foci,in which 2 patients were given hormonotherapy and small-dose radiotherapy after operation. All cases were cured and no recurrence were found after follow-up survey from halfa year to 2 years. Conclusions The disease is rare and call result in the loss of periodontal tissue in a short time.In its premature time.the disease can be effectively cured by focal excision with subgingival curettage and Chinese traditional medicine. If Deriodontal tissue has been invaded,Surgical excision and extraction of teeth is the main way.
